Dr. Shoaf Departure Letter

Dear patient,

We regret to announce that on September 30, 2015, Edwin H. Shoaf, Jr., MD, will retire from Novant Health Presbyterian Internal Medicine. While we are sorry to see Dr. Shoaf leave, we know you will join us in wishing him well.

Dr. Shoaf wants to thank you for the privilege of being your physician during his 36 years of caring for patients in Charlotte. He knows he will miss the practice of medicine, and he will especially miss his relationships with his patients. His greatest wish is for the continued health and wellness for all his patients.

As a patient of Novant Health Presbyterian Internal Medicine, you are important to us as well and we will work to make the transition as seamless as possible. Dr. Shoaf’s colleagues can provide you with the same remarkable care, and we are happy to help you find another physician within our clinic. Feel free to call the office so that we can ensure the transition is easy for you.

If you choose to receive care from a provider outside Novant Health, you will need to sign a request for us to transfer your records and we can help you with those arrangements.
